The fifth vial is poured out on the throne of the beast, and it brings dense darkness upon the kingdom. The suffering moves quickly from one intense issue to another. The pain of the earlier vials continues in the darkness, as does the continued rejection of God. Even though the pain is so intense that people gnaw their tongues, they continue to blaspheme God, and refuse to repent.
